ONBOARD - Lost Animal Recovery
ONBOARD is a service established, set up, maintained and ran through ACR.
ONBOARD is free of charge to any who wish to use it's services whether they register
with ACR or not.
ACR will document and maintain in the ONBOARD files the Micro-chip number, species,
breed, color/pattern/coat and last reported owners name, address, e-mail and telephone
number and will assist in contacting the person in our files in the hopes of returning the lost
animal to it's proper owner.
ACR will also document Tattoo's but please be advised that a single Tattoo can be used
a thousand times over and may make it impossible to be able to find the proper owner.
It's quick, easy and simple to have your animal put in our database. ACR does have
ONBOARD forms available that can be filled out or you can supply us the below information
by e-mail, fax, or mail.
- Legal Name of Owner(s)
- Address of Owner(s)
- Telephone number of Owner(s)
- E-mail Address of Owner(s)
- Animal's Name (Registered name if registered)(please submit copy of registration if
animal is registered) (Supply registered name is optionable)
- Name of Registration (if registered) (optional)
- Registration number (if registered) (optional)
- Species of Animal
- Breed of Animal
- Color/Pattern/Coat type of Animal
- DOB (registered DOB or approximate if not registered)
- DNA profile if DNA'd (must submit copy of DNA certificate) (optional)
- Micro-chip number
- Tattoo identification and location
- Medical alerts (if any) (optional)
- REQUIRED: SIGNATURE of SUBMITTER
TRANSFER OF OWNERSHIP ON AN ONBOARD LISTED ANIMAL:
In order to transfer ownership of an ONBOARD listed animal in ACR files the following
must be received:
- Bill of Sale, Transfer information or equivalant - SIGNED BY BOTH PARTIES
- ONBOARD Transfer card or equivalant
- REQUIRED - SIGNATURE OF BOTH PARTIES
A listed owner is the one who originally reports the animal into our databases.
Any change of the database requires proper documentation.
Disclaimer: ACR is not responsible for any animals that are not recovered nor is ACR
responsible for any animal that is retrieved by the wrong individual. ACR is also not
responsible for any inaccuarcies in it's files due to circumstances beyond it's control.
ACR is not responsible for any information that is falsely reported. ACR does not
guarantee the retrival of any animal. |
