Please find below the explanation of usage for each form

Litter Registration Application: 

To be used to register an individual litter between an ACR registered dam and sire  Is a 2 sided form and must have

the Puppy I.D. form on the back.

 

 

Litter Registration Application Puppy I.D. 

To be used in conjunction with the Litter Registration Application.  List each individual puppy's I.D. as requested. 

Back side of the Litter Registration Application

Litter Disposition Record:

To be kept for 7 years to document the sell of a puppy and the new owner.

Dual Registration:

To be used to dual register a dog that is already registered or has puppy papers from another acceptable registry.

Transfer of Ownership:

To be used to transfer an ACR dog from one documented owner to the next. 

American Pedigree Registry - APR forms

To be used to register an individual litter between an APR registered dam and sire  Is a 2 sided form and must have

the Puppy I.D. form on the back.

To be used in conjunction with the Litter Registration Application.  List each individual puppy's I.D. as requested.

Litter Disposition Record:

To be kept for 7 years to document the sell of a puppy and the new owner.

Dual Registration:

To be used to dual register a dog that is already registered or has puppy papers from another acceptable registry.

Hardship Registration:

This form is to be used to register a dog that does not currently have papers from another registry available. 

The dog can be of purebred or mixed breed lineage.   A pedigree can be submitted for lineage documentation.

Transfer of Ownership:

To be used to transfer an APR dog from one documented owner to the next.

Individual Puppy Registration Application:

To be used to individually register a puppy without applying for litter papers.  Parents must already be registered with

APR before this form can be used.   This form is used in place of applying for litter papers for those who wish to do so.

Requires use of Individual Puppy Registration Application Backside

 

Individual Puppy Registration Application Backside:

To be in conjunction with the Individual Puppy Registration Application.  It is the Fee and Address to the registry for

submission.  Failure to print this on the reverse of the Individual Puppy Registration Application will result in your

puppy buyer not having an address to send the paper in to for registration.

HYBRID Litter Registration - MIXED BREED:

To be used to register a litter from parents of separate breeds.  Parents must already be registered with APR before

this form can be used. 

This form must have the Litter Registration Application Puppy I.D on the reverse side.  Failure to include the reverse

side will result in paperwork being returned.

Biewer Yorkie Association - BYA forms

To be used to register an individual litter between an BYA registered dam and sire  Is a 2 sided form and must have

the Puppy I.D. form on the back.

To be used in conjunction with the Litter Registration Application.  List each individual puppy's I.D. as requested.
